This course seeks to help organizations differentiate between goal setting and person-centered goal setting. Participants will also learn the importance of documentation person-centered self-management goals and understand how to facilitate engagement to achieve better health outcomes for the individual.
This course is associated with Performance Activity PA_059, due 3/31/2018: “Demonstrate that at least 75% of indicated staff have been trained in person-centered self-management goal setting, documentation and review either through a CNYCC hosted training or other training meeting minimum CNYCC requirements.”

This course is very helpful in reminding CM of how to conduct client-centered self-management. It really helps CM to promote independence in client and to encourage them to be motivated to work towards reaching the goals they set for themselves.

Excellent, helpful. Using these tools will enable me to work more supportively with our clients. I have sat in on a self management class with my Pt.’s, through Cayuga County Health Network. I saw people finally participate, look forward to the next session, engage with others, not miss sessions, and leave with a sense of accomplishment and a sense of being part of a group or team. I love partnering with the expertise of CCHN, Jessica Soule, Amy Dickman, Georgette Jones.
