This course will provide guidance on how to identify patients who have repeated elevated blood pressure readings in the medical record but do not have a diagnosis of hypertension and schedule them for a hypertension visit.
This course is associated with Performance Activity PA_083, due 3/31/2018: “Implement the following in accordance to Group 6 – Hypertension Population Management as outlined in the latest iteration of the CNYCC Reference Guide for Cardiovascular Disease Management:
- Identify patients who have repeated elevated blood pressure readings in the medical record but do not have a diagnosis of hypertension and schedule them for a hypertension visit
- Train at least 75% of indicated staff to ensure effective patient identification and hypertension visit ”
