Patient Identification and Hypertension Visit Scheduling

This course will provide guidance on how to identify patients who have repeated elevated blood pressure readings in the medical record but do not have a diagnosis of hypertension and schedule them for a hypertension visit.

This course is associated with Performance Activity PA_083, due 3/31/2018: “Implement the following in accordance to Group 6 – Hypertension Population Management as outlined in the latest iteration of the CNYCC Reference Guide for Cardiovascular Disease Management:

  1. Identify patients who have repeated elevated blood pressure readings in the medical record but do not have a diagnosis of hypertension and schedule them for a hypertension visit
  2. Train at least 75% of indicated staff to ensure effective patient identification and hypertension visit ”
