Join Central New York Care Collaborative for a Patient Activation Measure/Coaching for Activation (PAM/CFA) Training session on Wednesday November 15, 2017 from 9:00 am to 3:30 pm. This will be an in-depth training session for “PAM Screeners” who will be utilizing the PAM and Coaching For Activation tool with patients/participants.
