Care Coordination

Training is designed not only for potential “patient navigators” but for all paraprofessional staff who do any type of care coordination. Frequent attendees are PCTs, MAs, Social Work Assistants, Community Workers, Customer Service representatives, Registrars and Unit Clerks.

Care Coordinator Training Program

Description

This program delivers 63 hours of comprehensive training on the core skills and competencies that are critical for the care coordinator role in today’s new models of care. Topics are presented in a series of nine bi- weekly, full day class room sessions. By going beyond the traditional lecture-based learning, this program strengthens students’ critical thinking skills by engaging them in rich discussion, individual exercises and group activities. Upon completion of this program, participants will be prepared to be strong, productive members of healthcare teams that provide coordinated, patient-centered care.
