      The Care Transitions Intervention (CTI) program can be found at this website: Care Transitions Program. In short, this program seeks to answer patients’ needs as they transition from hospital to home. It is an evidence-based intervention program that significantly reduces hospital readmission rates, reduces health-related costs, and put the power back into the individual in the form of self-management skills.

      On the website, you can use the Menu tab in the top left corner to search through various training opportunities, view documents related to CTI, and gain access to other related programs such as the Patient Activation Measure (PAM). The resources page can be found here: Tools & Resources.

      I would like to open the dialogue here in this community to discuss whether or not a program such as this bears any value in answering the needs of the LCHP region. Everything aside – the website provides free resources that may be worth looking into. Programs such as CTI are promising in the future of healthcare and are aligned with DSRIP initiatives to build a more efficient healthcare delivery system.
