 Strength in Numbers: There are over 70,000 members of our workforce, translating into almost 50,000 full time employees, many who live in the Bronx.

Our Projects

 2.a.i  Create Integrated Delivery Systems focused on Evidenced-Based Medicine/Population Health Management.

2.a.iii   Health Home At-Risk Intervention Program: Proactive management of higher risk patients not currently eligible for Health Homes through access to high quality primary care support services.

2.b.iii   Ed care triage at risk populations.

2.b.iv   Care transitions intervention model to reduce 30 day re-admissions for chronic health conditions.

3.a.i   Integration of primary care and behavioral health services.

3.b.i   Evidence-based strategies for disease management in high risk/affected populations – Cardiovascular Disease (adults only).

3.c.i   Evidence-based strategies for disease management in high risk/affected population – Diabetes (adults only). 

3.d.ii  Expansion of asthma home-based self-management program.

4.a.iii  Strengthen Mental Health and Substance Abuse Infrastructure across Systems.

Our Values
The BPHC partners are fully committed to the principle that DSRIP is about patients, not institutional power or growth. The coalescing around SBH, a well-respected community hospital, as the lead applicant for this DSRIP demonstrates diverse organizations’ commitment to work together to create a balanced governance structure prioritizing patients and their health outcomes. Support has come from organizations including Montefiore, one of the largest and most successful academic medical centers in NYS, five FQHCs, leading long-term care and developmental disability providers, three health homes, substance abuse agencies and a diverse set of community-based organizations, including Acacia Network, RAIN, H.O.G.A.R, and Care for the Homeless.
